Before you submit this digital Photo Identification Card application form you will need the list below:

1 . Manager approval.
You will need WRITTEN approval from your manager, authorising you to apply for the Photo ID card. You will need to upload this in PDF format.

2. Copy of your signature.
Black ink only and must be on white background.

3 . A portrait photo of yourself.
NOTE: White background, portrait, JPEG, similar to passport style below, must be face on. Professional photos will not be accepted. Photos that do not meet the above criteria will be declined and the application will need to be resubmitted.”

These are the expected requirements and guidelines for Qld Government ID Card Photographs:

Image background must be light grey, cream or white
Subject must be face on to the camera. No side poses or head tilted down or up
Glasses can be worn on face, provided frame is not covering any part of the eyes and there is no reflection on the lenses
Tinted glasses or sunglasses are not acceptable
Frames must not obscure any part of the face
No hats or glasses to be worn on top of head
Only nationality or religious head-covering is acceptable, but must not cover the face
Photo must capture image from head to mid torso (we crop to the correct size)
Only passport style images acceptable
Images cut from group photos or recreational images are non-compliant
Even shadowless lighting on the face
No sepia or image modifications
Image uploaded must be current within the last two (2) years
Image size to be no larger than 1mb
Image file type must be a jpg file only

4 . Cost Centre/WBS Number.
You will be required to input your Cost Centre/WBS Number as part of the submission. Please have this ready.

Property and Facilities Management is collecting staff information to provide a photographic identification card through the Queensland Police Service (QPS). All personal information will be securely stored and only accessible by authorised officers and then transferred electronically to the QPS for production of the card. The personal information will otherwise not be disclosed to third parties without consent.
